서론

뇌혈관 내피세포의 손상을 유발하는 위험인자나 질환들은 혈관 내 혈전생성과 뇌졸중 위험도를 증가시킨다. 뇌졸중의 주요 원인인 죽상경화(atherosclerosis)는 혈관 내피세포의 손상에 의해 발생하는 만성염증성 질환이다.1

 

근거

많은 코호트 연구에서 hs-CRP(high sensitivity C-reactive protein) 증가와 심뇌혈관질환 및 뇌졸중발생과의 관련성이 보고되었으며, 건강한 성인에서 hs-CRP 수치가 증가함에 따라 뇌졸중 발생 위험도가 2-3배 증가하는 것으로 보고되었다.2,3 임상연구에서 스타틴(statin)은 관상동맥 내죽상경화의 진행과 심뇌혈관질환의발생을감소시켰는데, 이는hs-CRP 수치의감소와밀접한관련성이있었다.4-6

hs-CRP가 증가되어 있지만 고지혈증이 없는 환자군에서 스타틴을 투여한 경우 심뇌혈관질환의 발생은감소하였지만, 뇌졸중 발생의 감소는 분명하지 않았다.7 죽상경화성 심뇌혈관질환과 염증반응 및 관련된 대리 표지자(surrogate marker)와의 관련성에 대하여 많은 연구가 진행되고 있으나, 환자치료에 적용하기 위해서는 더 많은 연구가 필요하다.

외국의 권고사항

1. Currently, no evidence supports the use of high-sensitivity CRP (hs-CRP) screening of the entire adult population as a marker of general vascular risk (ASA: LOE IV, GOR C).

2. Aggressive risk factor modification is recommended for patients at high risk for stroke given exposure to traditional risk factors regardless of hs-CRP level (ASA: LOE Ia, GOR A).

In agreement with AHA/CDC(Center for Disease Control and Prevention) guidelines, hs-CRP can be useful in considering the intensity of risk factor modification in those at moderate general cardiovascular risk on the basis of traditional risk factors (ASA: LOE III, GOR B).

국내의 권고사항

1. 모든 성인에서 심뇌혈관질환의 위험도를 평가하기 위한 선별검사로 hs-CRP를 측정할 근거는 부족하다. (근거수준IV, 권고수준C)

2. 뇌졸중 고위험군에서는 hs-CRP 수치와 관계없이 적극적인 위험인자의 조절이 필요하다. (근거수준Ia, 권고수준A)

3. 심뇌혈관질환의 중등도 위험군에서 위험인자의 치료 강도를 결정하는데 hs-CRP 수치가 고려될 수 있다. (근거수준III, 권고수준B)
