Regulation of Caveolin-1 Expression Determines Early Brain Edema After Experimental Focal Cerebral Ischemia

뇌경색으로 사망하는 경우, 대부분 허혈 뇌조직의 부종과 출혈성 변화가 사망의 원인이며, 이를 조절하는 것은 뇌경색의 심각한 예후를 개선시키는데 도움이 된다. 최근 국내 연구진이 뇌허혈모델에서 cerebral edema와 blood-brain barrier (BBB) disruption의 기전과 관련된 연구논문을 발표했다.
Vasogenic cerebral edema와 hemorrhagic transformation은 모두 BBB의 손상에 의해 일어나게 된다. 그러므로, vascular integrity는 뇌허혈의 치료에 있어 매력적인 대상이다. 그러나 임상에서 BBB integrity를 다루는 치료방법은 없다. BBB breakdown의 기전을 아는 것이 치료와 뇌졸중 예후에 의미 있는 영향을 줄 것이다.
본 연구에서는 ischemic insult 후의 뇌부종에 대한 Caveolin-1 (Cav-1)의 역할을 알아보았다. Photothrombosis로 mice에서 뇌허혈을 유발한 후 BBB disruption의 정도를 시간에 따라 확인하고, immunoblotting으로 Cav-1 발현정도를 측정하여 시간에 따른 변화 및 BBB disruption과의 연관성을 보았다. Cav-1 protein의 gene disruption, re-expression의 방법으로 뇌허혈에서 cerebral edema와 BBB permeability에 대한 Cav-1의 역할을 확인했다.
Cav-1 expression과 BBB disruption간에는 유의한 연관관계가 있었다. Cav-1 deficient mice에서 뇌허혈 유발 후 BBB disruption이 wild-type mice에 비해 더 증가했으며, 증가했던 brain edema가 lentiviral-mediated re-expression에 의해 개선되었다. 또한 Cav-1 deficient mice는 wild-type에 비해 tight junction protein이 더 많이 degradation되었고, matrix metalloproteinase의 proteolytic activity가 더 높았다. Cav-1 deficient mice에서 Cav-1을 re-expression했을 때 tight junction protein expression이 회복되고, MMP proteolytic activity도 감소했다.
본 연구결과는 cav-1이 tight junction protein의 발현과 MMP activity를 조절하여 BBB permeability에 중대한 영향을 미친다는 것을 시사한다. 이는 다른 ischemic model에서도 확인이 필요하나, cerebral edema및 BBB permeability에 대한 치료 target이 될 수 있는 가능성이 있다.
