Effect of blood pressure-lowering agents on microvascular function in people with small vessel diseases (TREAT-SVDs):a multicentre, open-label, randomised, crossover trial

Introduction

고혈압은 뇌소혈관질환(cerebral small vessel disease)의 가장 주요한 위험인자이다. .

Objectives

이 연구는 뇌소혈관질환을 가진 사람에게서 고혈압약제 계통에 따라 미세혈관기능에 미치는 영향이 다른지 확인하고자 하였다.

Methods

ž   연구방법: multicenter, open-label, randomized crossover trial with blinded endpoint assessment, 5 specialist centers in Europe

ž   참가자: Symptomatic sporadic small vessel disease (SVD) 또는 CADASIL + 고혈압 치료 대상

ž   그룹 및 약물 투여

-         Amlodipine : Losartan : Atenolol = 1:1:1

-         2주간 washout period 이후 4주간 단일 약물 투여

ž   Primary Endpoint

-         정상 white matter brain 에서 hypercapnic challenge에 대한 blood oxygen level-dependent MRI를 이용하여 측정한 cerebrovascular reactivity (CVR) 변화

-         washout 종료에서 각 치료제 투여 기간 종료시점간 측정

Results

ž   75명의 sporadic SVD 환자 (평균 64.9세) 와 26명의 CADASIL 환자(평균 53.1세)등록 되었고 최종 분석에는 79명의 참가자(sporadic SVD 62명, CADASIL 17명)가 참여

ž   Sporadic SVD환자에서 CVR 변화는 약물 간에 차이가 없었으나(amlodipine의 CVR 평균 변화 1.8 × 10^–4%/mm Hg [SE 20.1; 95% CI -37.6 to 41.2], losartan의 CVR 평균 변화 16.7 × 10^–4%/mm Hg [SE 20.0; -22.3 to 55.8], atenolol의 CVR 평균 변화 -7.1 × 10^–4%/mm Hg [SE 19.6; -45.5 to 31.1]; p=0.39), CADASIL 환자에서는 CVR의 변화가 다르게 나타났다. (amlodipine의 CVR 평균 변화 15.7 × 10^–4%/mm Hg [SE 27.5; -38.3 to 69.7], losartan의 CVR 평균 변화 19.4 × 10^–4%/mm Hg [SE 27.9; -35.3 to 74.2], atenolol의 CVR 평균 변화 -23.9 × 10^–4%/mm Hg [SE 27.5; -77.7 to 30.0]; p=0.019). CADASIL 환자에서 amlodipine은 atenolol과 losartan과 비교하여 모두 CVR이 개선되는 소견을 보여주었다.

Conclusions

4주 동안의 amlodipine, atenolol, losartan 치료는 sporadic SVD 환자의 미세 혈관 반응에 차이를 나타내지 않았으나, CADASIL 환자에서는 치료 효과에 차이가 있었다. SVD 환자의 고혈압약제 종류가 임상 결과에 다르게 작용하는지에 대하여는 추가적인 연구가 필요하다.

Comment

임상적으로 Cerebral small vessel disease 환자에서 우선적인 고혈압약제 선택에 대한 근거는 여전히 부족하며 추후 연구가 필요하다.
