서론

경구용 피임제가 허혈성 뇌졸중을 유발할 수 있다는 근거는 고용량의 여성호르몬이 함유되어 있는 1세대 경구용 피임제를 대상으로 시행된 초기의 연구에서 나온 것이다. 저용량의 여성호르몬이 함유되어 있는 최근의 경구용 피임제는 뇌졸중의 위험을 증가시키지 않는 것으로 보인다.

 

근거

메타분석1에 의하면 저용량의 여성호르몬이 함유되어 있는 최근의 경구용 피임제는 뇌졸중의 위험을 증가시키지 않았다. 4개의 코호트 연구 메타분석에 의하면 경구용 피임제의 뇌졸중 발생 대응비(odds ratio)는0.95(95% CI, 0.51-1.78)로 유의한 차이가 없었다. 36세 이상의 연령, 흡연, 고혈압, 당뇨, 편두통, 혈전 색전증의 과거력 등이 있는 경우 경구용 피임제를 사용하면 뇌졸중의 위험이 증가할 수 있다.2-6 이러한 위험 인자를 가지고 있는 여성의 경우 경구용 피임제를 피하는 것이 바람직하며, 부득이 하게 경구용 피임제를 복용해야 하는 경우 동반된 뇌졸중의 위험인자들에 대한 철저한 치료를 시행하는 것이 바람직하다.7

외국의 권고사항

1. The incremental risk of stroke associated with use of low dose oral contraceptives in women without additional risk factors appears low, if one exists (ASA: LOE III, GOR B).

2. It is suggested that oral contraceptives be discouraged in women with additional risk factors (eg, cigarette smoking or prior thromboembolic events) (ASA: LOE III, GOR B).

3. For those who elect to assume the increased risk, aggressive therapy of stroke risk factors may be useful (ASA: LOE IV, GOR C).

국내의 권고사항

1. 뇌졸중 위험 인자가 없는 여성에서 저용량의 경구용 피임제가 뇌졸중 위험을 증가시킬 가능성은 낮다. (근거수준III, 권고수준B)

2. 흡연이나 혈전색전증 등의 뇌졸중 위험인자가있는 여성에서는 경구용 피임제 복용을 피하는 것이 바람직하다. (근거수준III, 권고수준B)

3. 뇌졸중 위험인자가 있지만 경구용 피임제를 복용해야 하는 경우 위험인자에 대한 철저한 치료를 시행하는 것이 바람직하다. (근거수준IV, 권고수준C)
