서론

규칙적인 신체활동에 의해 조기 사망과 심뇌혈관질환 사망이 감소하며, 이는 뇌졸중 예방에도 도움이 된다. 이 결과는 신체활동이 혈압을 낮추고, 혈당을 조절하며, 체중을 감소시키는 과정에 의해 나타나는 것으로 알려져 있다.

 

근거

신체 활동의 증가는 허혈성 뇌졸중을 감소시킬 수 있는데, Oslo Study, Honolulu Heart Program, 그리고 Framingham Heart Study 등은 남자에서의 예방 효과를,1-3 그리고 Nurses’Health Study 와 Copenhagen City Heart Study 등은 여자에서의 예방 효과를 증명하였다.4,5 이전의 연구에서는 운동량에 따른 양-반응 관계가 명확하지 않았으나,3 최근의 연구에서는 운동량이 많은 경우, 운동량이 적거나 중등도인 경우에 비하여 허혈성 뇌졸중 발생이 유의하게 감소된다고 보고하고 있다.6 하지만 노인에서 격렬한 운동량을 보이는 군의 수가 많지않음을 고려해야 한다.

신체활동에 의한 뇌졸중 발생의 감소 효과는 혈압을 떨어뜨리고, 체중을 감소시키며, 혈당을 조절하고, 심뇌혈관질환의 다른 위험인자들을 조절하는 것으로 일부 설명될수 있다.

미국 질병관리본부(CDC)와 국립보건원(NIH)에서는 중등도의 운동을 가능한 매일 30분 이상 시행할 것을 권고하고 있다.7

외국의 권고사항

1. Increased physical activity is recommended because it is associated with a reduction in the risk of stroke (ASA: LOE III, GOR B). Exercise guidelines as recommended by the CDC and the National Institutes of Health of regular exercise (≥30 minutes of moderate-intensity activity daily) as part of a healthy lifestyle are reasonable (LOE III, GOR B).

2. Regular physical activity is recommended (EUSI: LOE III, GOR B).

국내의 권고사항

1. 뇌졸중의 일차예방을 위해 신체활동을 늘리는 것이 필요하다. (근거수준III, 권고수준B).

2. 규칙적인 운동(매일 30분 이상의 중등도 운동)이 뇌졸중 예방에 도움이 될수 있다. (근거수준 III, 권고수준B)
