폐경후 호르몬치료 (Postmenopausal hormone therapy) - 발간: 2009. 10

서론

여성에서 뇌졸중발생은 폐경 이후 급격히 증가한다. 폐경후 호르몬 치료는 실험연구 및 관찰연구들 에서 심뇌혈관질환 예방 및 뇌졸중 중증도 감소 등의 효과가 기대되었으나, 임상시험에서는 오히려 뇌졸중의발생을 증가시키는 것으로 보고되었다.

 

근거

59,337명의 폐경후 여성을 16년간 추적 관찰한 Nurses’Health Study1에서 에스트로젠 투여와 뇌졸중 발생간의 뚜렷한 관련성은 관찰되지 않았다. Heart and Estrogen/progestin Replacement Study II(HERS II) 연구2에서 건강한 폐경후 여성에서 호르몬 치료는 허혈성 뇌졸중의 위험을 증가시키는 경 향이 관찰되었다. Women’s Health Initiative3는 폐경후 여성에서 호르몬 치료의 심뇌혈관질환의 일차 예방 효과를 평가하기 위하여 시행되었다. 본 연구 결과, 1년에 10,000명을 대상으로 호르몬 치료를 시행하는 경우 오히려 8건의 뇌졸중이 증가하였으며, 자궁 적출술을 시행 받은 환자에서도 뇌졸중 발생은 호르몬 치료를 받은 피험자에서 유의하게 증가하였다.4 (RR 1.39; 95% CI, 1.10-1.77) 28개 임상 연구의 메타분석결과에서도 폐경 후 호르몬 치료에 의해 뇌졸중 발생은 유의하게 증가하였고, 호르 몬치료를 받고 있던 경우에발생한 뇌졸중의 예후도더나쁜 것으로 확인되었다.5

외국의 권고사항

1. It is recommended that postmenopausal hormone therapy (estrogen with or without a progestin) not be used for primary prevention of stroke (ASA: Class III, Level of Evidence A) (EUSI: Level I).

2. The use of hormone replacement therapy for other indications should be informed by the risk estimate for vascular outcomes provided by the reviewed clinical trials.

3. There are not sufficient data to provide recommendations about the use of other forms of therapy such as selective estrogen receptor modulators.

국내의 권고사항

1. 뇌졸중의 일차예방 목적으로 폐경 후 호르몬 치료를 하는 것은 권장하지 않는다. (근거수준 Ia, 권고 수준 A)

2. 다른 적응증으로 폐경 후 호르몬 치료가 필요한 경우 심뇌혈관질환의 위험성 증가에 대한 설명과 상담이 필요하다. (권고수준GPP)
