체성감각의 보존 정도가 뇌졸중 이후의 재활치료 효과의 차이를 설명한다.

본 연구는 뇌졸중 환자에서 somatosensory system 및 motor system 의 온전성 (integrity) 이 proprioceptive feedback 증진 목적으로 설계되어 있는 robotic therapy 의 치료효과에 영향을 줄 것이라는 가설을 규명하는 것을 목적으로 삼고 있다.

만성 뇌졸중 30명을 대상으로 연구가 진행되었고, somatosensory system, motor system 의 온전성을 정량적으로 평가하기 위해서 behavioral performance, neural injury, neural function 을 평가하였는데, 각각 항목의 평가 도구는 다음과 같다. 1) behavioral performance; Box and Block test, Action Research Arm Test, Fugl-Meyer Scale (upper extremity), Nine Hole Peg Test, Finger Tapping test, Edinburgh Handedness Inventory, 2) neural injury; structural MRI (T1-weighted images, T2 fluid-attenuated inversion-recovery images) 을 통해 infarct volume, gray matter injury 정도, white matter injury 정도를 계산. 3) neural function; functional MRI 을 통한 cortical function 확인 뿐만 아니라 fMRI 영상을 기반으로 ROI-ROI approach 를 통해 cortical connectivity 확인. EEG 를 통해 iM1, iS1, iS2 구역에서의 high-beta frequency band 의 coherence 를 확인.

30명의 뇌졸중 환자들은 3주간 finger movement 향상을 위한 robot-based therapy 를 제공받았다. 치료 기간 후, 이중 9명의 환자들은 Box and Block test 로 평가하였을 때 MCID (minimal clinically important difference) 를 초과하는 치료효과를 보였고, 8명의 환자들은 효과가 전혀 없었다. 상기한 baseline 평가 지표들을 활용한 모델을 활용하여 이와 같은 치료효과의 차이를 설명해 본 결과, structural MRI 상에서 확인된 total sensory system 의 손상, functional MRI 을 통해 계산된 S1 laterality index 및 iM1-iS2 functional connectivity, 그리고 EEG 를 통해 확인된 iS1-cS1 coherence 가 본 치료효과의 차이를 설명하는 (통계학적으로) 유의미한 변수임을 알 수 있었다. 더 나아가 structural MRI 상에서 확인된 total sensory system 의 손상과 functional MRI 상에서 확인된 iM1-iS2 functional connectivity 를 활용한 multivariate model 이 치료효과의 56% 를 설명하는 것으로 확인하였다.
