Albumin-binding MR contrast를 이용한 vulnerable atherosclerotic plaque의 예측

Albumin-binding MR contrast를 이용한 vulnerable atherosclerotic plaque의 예측

Atherosclerosis는 뇌경색과 심근경색의 매우 중요한 원인 중 하나이며, atherosclerosis로 인한 thromboembolic event 에서 unstable plaque의 rupture가 주요 원인으로 작용한다. 초음파나 MRI를 이용하여 unstable plaque을 구분하고자 하는 시도는 계속되고 있으며, 최근 albumin-binding MR contrast (gadofosveset)를 이용한 T1 mapping 영상으로 vulnerable plaque을 확인한 연구결과가 발표되었다.

연구자들은 atherosclerosis rabbit 모델에서 plaque rupture를 유도하여 vulnerable plaque과 stable plaque의 MRI 결과, histology 결과를 분석했다. Cholesterol diet와 balloon injury를 통한 endothelium denudation으로 atherosclerosis를 유도한 후, Russell’s viper venom과 histamine으로 plaque rupture를 trigger 했다. trigger이후의 thrombus생성 여부에 따라, trigger 전의 plaque을 stable plaque과 rupture-prone plaque으로 나누었다. Atherosclerosis 유도 3주, 12주째에 MRI 영상검사를 시행하여, plaque area, vascular permeability, vasodilation, stiffness를 확인하였고, 그리고 rupture를 trigger 한 후에 thrombus 생성여부를 확인했다. 면역조직화학검사 및 전자현미경 등의 방법으로 영상검사결과와 실제 조직검사결과의 일치 여부를 검사했다.

실험에 이용된 모든 rabbit에서 atherosclerosis가 유발되었고 60%에서 thrombus가 생성되었다. gadofosvest의 주입 후 vascular permeability를 측정한 결과 Rupture-prone plaque에서 stable plaque에 비해 vascular permeability가 더 높았다. 면역조직화학검사에서도 ruptured plaque에서 vessel wall albumin이 증가하고 microvessel density가 증가했다. 이는 stable plaque에서도 normal vessel에 비해 증가했으나, rupture-prone plaque에서 가장 뚜렷하게 나타났다.

연구자들은 albumin-binding contrast를 이용한 T1 mapping이 비침습적인 방법으로 atherosclerosis와 plaque instability와 관련된 vascular permeability의 변화를 측정하기 위해 사용될 수 있다고 제안하였다.
